Preparing for Community Living
A community discharge coordinator liaisons between inpatient psychiatric services and community housing providers. The goal is to facilitate a seamless transition from hospital to community living, overcoming barriers by providing one to one individualized support. The program provides a community-based assessment of skills and services to facilitate moving referred clients into an optimal community setting.
Key Components:
- Engage referred patients while on the inpatient units, developing a relationship and a person centered approach to both discharge and transition planning issues
- Provide consultation to inpatient staff regarding potential discharge options
- Facilitate referral process into community placements via transitional visits
- Review O/T assessments to inform of community placement considerations
- Review barriers to discharge and research alternative strategic approaches to overcome barriers
