Scattered Site Case Management
Continuum provides varying degrees of case management to clients residing in their own apartments including assistance with locating appropriate and affordable housing and developing skill maintenance and autonomy with case management services at least once per week.
Key Components:
Independent client-held leased apartments located in New Haven, Hamden, Woodbridge, or Bethany- Enhancement of support networks
- Program capacity: 25
- Approximate overall staff to client ratio of 1 to 8
