Scattered Site Case Management
The Permanent Supportive Housing Programs are a collaborative effort of Continuum of Care Inc., Continuum Home Health, Inc., and the Housing Authority of New Haven (HANH). Support services are offered in three HANH high rises including McQueeney Towers, Crawford Manor, and William T. Rowe.
Each case management program is funded in part by the State of Connecticut Department of Mental Health and Addiction Services (DMHAS) through an initiative called Planning and Implementing Housing Options for Long Term Success (PILOTS) funding and by the Housing Authority of New Haven.
Services provided include wrap around case management services to individuals at risk of being of being homeless through eviction, and who have either a dual diagnosis of mental illness and substance abuse, or a history of substance abuse.
Case managers meet with clients weekly to provide assistance in activities of daily living, money management, transportation, entitlements, housing issues, therapeutic recreational groups and activities, and individualized clinical treatment planning. The daily structure includes participation in meditation groups, dual diagnosis groups, symptom management, substance abuse prevention, and twelve-step meetings.
