DIVERSION SERVICESnursing home discharge and diversion services (obra)
The purpose of this program is to ensure that nursing home placements for DMHAS clients are necessary, appropriate, and safe. Preadmission Screening Resident Review (PASRR) is an integral part of the program.
Focusing on two specific goals, the program will reduce inappropriate admissions of DMHAS clients to nursing homes and monitor admissions that do occur for appropriate placement and/or eventual discharge back to the community with support services. To accomplish these goals, DMHAS funds six Nurse Clinicians, working directly with community providers, nursing homes, and hospital discharge planners, both on inpatient units and in emergency rooms.
