ACUTE SERVICES

Community Transitional Services (CTS-Step-Down)

CTS is a collaboration with a private psychiatric hospital and serves as a step down from acute inpatient hospitalization where individuals can stay in a safe and stable short-term location during their transition to their next level of care. Residents are provided a range of services, including case management, psychoeducational groups, psychiatric rehabilitation, intensive life skills building, and housing resource coordination.

Community Integration Evaluation Program (CIEP)

CIEP provides in-depth evaluations and assessments of individuals with severe and persistent mental illness who have been institutionalized with significant barriers to community living. The program has a two-phase approach to community reintegration. During the first phase, staff engage individuals in social and recreational activities, both in the inpatient unit and out in the community. During the second phase, staff conduct comprehensive assessments of their basic life skills. The goal is to assess each individual’s strengths, abilities, needs, and preferences when looking at placing him or her in supported independent living in the community.
