Continuum of Care, Inc.’s mission is to enable people who are challenged with mental illness, intellectual disabilities, addiction, or homelessness to rebuild a meaningful life and thrive in the community.
Mental Health Services
Continuum of Care supports a recovery-oriented system of care that addresses clinical or symptom stabilization, as well as overall community integration for people challenged with behavioral health issues. Continuum's residential programs serve residents of Region 2, Catchment Area Council (CAC) 7 including the towns of New Haven, Woodbridge, Bethany, and Hamden. Crisis services are also available to residents of CAC 5, 6, and 8 covering the Valley and the Shoreline. Each program operated by Continuum is designed to promote the acquisition of life skills while respecting and fostering client choice. We provide culturally competent services while individualizing goals and objectives based on each client's preferences.
